Privacy Practices and Forms
DWEL-AZ is a statewide data warehouse that strengthens care coordination for individuals experiencing homelessness in Arizona. With permission from the individual receiving services, participating agencies securely share and link information to streamline eligibility, improve services, and better understand community needs.
Privacy is a top priority. All data is protected through encryption, password safeguards, restricted access, and other security measures.
The following information will be shared:
Identity Verification Details
Personal identifying information such as name, healthcare identification information, and date of birth. Social Security Numbers are used if a healthcare identification number is not available; however, Social Security Numbers are not viewable by staff and are only used in the system to verify the identity of an individual.
Personal Identity Factors
Demographic information such as race, ethnicity, and gender.
Support Qualifiers
Information about you that may help in locating housing and other resources, such as veteran status or whether you or a member of your family has a disabling condition.
Housing History
Information about your history of homelessness such as where you have been living and how we can reach you.
